Winter to Spring Skincare Tips

The warmer months bring more moisture in the air, which can give your skin a lovely, healthy glow. However, the increased temperatures can also aggravate skin concerns like rosacea, eczema, hyperpigmentation, sun sensitivity, and oiliness. With longer days comes more sun exposure, which heightens the risk of sun damage to your skin.

"A solid routine should be followed for all seasons and all climates. However, just as you swap out your winter wardrobe for your spring wardrobe, you should change your skincare routine with the seasons," says Natalie Abouchar (NP) Nurse Practitioner and Director of Privée Clinic. "Everyone’s skin is different, depending on their skin type and where they live. There are a couple of changes I recommend when transitioning from winter to spring. Here are my top spring skin tips”.

Natalie’s Top Spring Skin Tips

SPF

Don't leave home without it, it’s the most important. A broad-spectrum sunscreen SPF 30 or higher on all skin that’s exposed to the sun. Don't forget about your hands, feet, ears, and lips. Our favourite facial SPF at the clinic is Mesoprotech Melan 130, tinted and perfect to protect against pigmentation .

It’s also important that sunscreen is re-applied every couple of hours, so make sure to pack extra sunscreen for those longer warmer days outdoors.

Use lighter weight skincare products

When you start noticing that your skin is producing more natural oils in spring, many people may think it’s appropriate to eliminate moisturiser altogether, but that’s a common mistake.

In the Spring, I generally remind all my clients that their skincare routine should be a little more lightweight. Consider combining your sunscreen and moisturiser for a lighter weight alternative to avoid the clogging of pores on your face. Also, the heat and humidity means you can swap out your heavier cleanser in favour of a foaming option. For example, our favourite cleanser at the Clinic is the Advanced Cleanser from Dermaceutic.

Vitamin C Cream

Vitamin C is great to use year-round, but super important in the spring and summer. Vitamin C is proven to brighten and even your complexion as well as providing significant anti-ageing benefits. It can also help with collagen production. Collagen is an essential protein required for healthy youthful looking skin. Put a few drops on your skin between cleansing and moisturiser. My favourite Vitamin C product is CE Ferulic which is a potent antioxidant, pigment inhibitor and skin brightener.

Exfoliate

Winter can leave your skin feeling dry and flaky, accumulating dead skin cells that dull your complexion. Spring is the perfect time to remove this layer through gentle exfoliation. Exfoliation is a necessary step to maintaining that dewy and healthy glowing skin. It's especially helpful if you suffer from acne, blocked pores or pigmentation. Chemical exfoliants work best; look for products such as Salicylic, Mandelic, Kojic and Azelaic acid. I recommend starting slow by using them 3 times per week and working up to using them every night. The Radiance brightening cream by Dermaceutic is a good quality exfoliator which has a cocktail of different acids making it great for skin brightening, reducing pigment spots for a unified and radiant complexion.

Wear sun-protective clothing

Wear sun-protective clothing, wide-brim hats and sunglasses. Expose as little of your body as possible to direct sunlight. Consider wearing long sleeved shirts where appropriate, sit underneath a large umbrella and avoid the midday sun if you can.

The Exilis Ultra 360 is a non-invasive device that uses a powerful combination of radiofrequency and ultrasound to tighten and target both superficial and deep problem areas. Apply cold compresses

Six months out from your wedding

Starting your wedding skincare routine six months in advance will ensure you reap all the benefits of your treatments and products. Whether you're a super organised bride or planning last-minute, this guide has you covered.

Start using Retinol (if you’re not already)

Retinol is a form of vitamin A that belongs to the retinoid family. It stimulates skin cell renewal and encourages collagen production to create a thicker and plumper layer of skin. Overall improving the appearance of fine lines, wrinkles, discoloration, and skin texture. It can even treat acne prone skin.

Retinol can be quite potent for some people. So it's important to start slow and gradually build up your usage. Start by using it - only at night - 3 times a week initially and gradually increase the frequency to every night once you see that your skin tolerates it.

Natalie Abouchar says “I’ve been using Retinol for a couple of years now, I started slow and built up to every night. Firstly I cleanse, ensuring the skin is completely dry, then apply a pea size amount of Retinol all over my face. I will use the same amount on my neck and chest, then apply a thick moisturiser. It’s a quick and effective routine to minimise pores, improve texture and keep the skin exfoliated.”

Four months out from your wedding

Try laser treatments

If your primary concern is eradicating sun spots or hyperpigmentation before your wedding, whether that be on your face or body, Intense Pulsed Light (IPL) might be an effective treatment for you.

Intense Pulsed Light (IPL) is mainly for people with fairer skin types on the Fitzpatrick scale 1-3, who want to treat sun damage and clear uneven colour from their skin such as red and brown tones. It treats redness, rosacea, sun damage, freckles and any non melasma pigmentation. IPL helps to even out skin tone and stimulates the production of collagen.

IPL uses Broad spectrum wavelengths of light in each pulse to target red and brown chromophores in the skin and the inflammatory pathway and stimulates the production of collagen.

“Depending on how severe the pigmentation is, I will generally do about three or four IPL treatments before the wedding day in order to see optimal results,” says Natalie. Because of this, I recommend coming in for a consultation about four to six months in advance of your wedding. This gives you enough time to assess how your skin reacts to the treatments, and gauge how long the downtime will be for your particular skin type, while also having the flexibility to change around treatments as needed.

Chemical peels

Don’t start any new treatments or products

During the final weeks leading up to your wedding day, you should avoid trying any new products or treatments in order to avoid an irritation or flare-up. “The last thing you need a few weeks before your wedding is a bad reaction to something new you tried. So just keep using things that work for you, now is not the time to change anything,” explains Abouchar

One Week Before the Wedding

Focus on hydration

“In order to avoid any possible skin irritation on the day of your wedding, and depending on your skin, it may be effective that you stop or reduce using exfoliators or retinol about a week prior to your wedding. Acids and retinol can trigger dry patches and irritation. Holding off the week before is an easy way to avoid a crisis the day of your wedding,” says Abouchar. “Instead you could opt for hydrating and calming serums or moisturisers to soothe and quench the skin. And drink lots of water!”

As your wedding day approaches, it's also important to prioritise sun protection to prevent sun damage. Use a broad-spectrum sunscreen with an SPF of 50 everyday, and reapply it throughout the day as needed.

7 Ways to Reduce Open Pores

The skin is the largest organ in the human body and contains millions of pores, with most of these pores not visible to the human eye. Pores are tiny openings on the surface of the skin and are connected to either sweat glands or oil glands. The function of a pore is to allow passage of oil or sweat to the surface of the skin, which allows the skin to cleanse itself.

Enlarged pores can lead to an aged appearance. There are several possible causes of enlarged pores. They include:

Excess oil (sebum) production
Ageing (there is less collagen supporting the surrounding pore, causing it to appear more open)
Skin type and genetics
Sun damage (causing a reduction in collagen)

While it is not possible to completely eliminate pores as they are needed for normal skin function, we can reduce them in size. Most of the treatments used to minimise the look of pores work to either reduce sebum production and/or increase collagen in the skin.

Here are 7 of the BEST tips or treatments for reducing open or enlarged pores.

1) Regular cleansing of your skin.

Washing your face, both morning and night, is easy to do and helps wash away oil and dirt from your pores. Before going to sleep, it’s important to remove your makeup because sleeping in makeup overnight can cause pores to become blocked due to a build up of oil and bacteria. For oily or acne prone skin, cleansing with a salicylic acid based cleanser will help to gently exfoliate and unclog trapped debris and oil, reducing the formation of blackheads and skin blockages. This can be used on your face, chest and back, or any areas that are prone to blockage.

2. Exfoliate

Exfoliating your skin is especially helpful if you suffer from blocked pores, acne or pigmentation. Chemical exfoliants work best, look for products with Salicylic, Mandelic, Kojic and Azelaic acid. We recommend starting slow by using them 3 times per week and working up to using them every night. The Radiance brightening cream by Dermaceutic is a good quality exfoliator which has a cocktail of different acids making it great for reducing the look of enlarged pores, skin brightening, and reducing pigment spots for a unified and radiant complexion.

3. Retinol

Retinoids are a must when trying to reduce the look of pores. They work by increasing cell turnover, reduce sebum production and upregulate collagen production in the skin. The multi modality effects of Retinol make it a winner. The trick with Retinol is to introduce it slowly to your skin care routine, as using too much too soon can cause irritation and dry skin. We recommend using it in the evening 3 times per week and increasing it slowly to every second night, then every night over the period of 4-6 weeks.

4. Niacinamide

Niacinamide is an antioxidant and B vitamin which when used topically, has been shown to reduce inflammation in the skin and consequently reduce the size of the pore. Niacinamide can also increase the production of ceramides and lipids, reduce sebum production and improve skin elasticity by increasing the collagen production. Niacinamide works wonders when used in conjunction with Retinol and is very beneficial for most skin types including acneic skin.

5. Chemical peels

A chemical peel removes the outer layer of the skin in a controlled manner via the application of acids. There are different depth peels - superficial, medium or deep, all which give varying lengths of recovery time. Generally superficial peels have minimal down time, while medium depth peels may take 7 days to recover. Deep peels are rarely used these days but recovery time may be 2-4 weeks.

Following a superficial chemical peel, the skin may be slightly pink, however it will feel smooth and exfoliated, and your pores will be clearer! A chemical peel can also diminish fine lines, wrinkles, improve dull skin, reduce pigmentation and improve texture.

Medium depth peels include Cosmelan and TCA (Trichloracetic acid) peels. These work by aggressively exfoliating the skin, while also increasing the production of collagen via the body’s inflammatory response. The peel stimulates the skin fibroblasts to start making more collagen in response to the trauma. The improved pore size from the increase in collagen production can be seen from the 6 week mark. The exfoliative effect of the peel will improve the look of pores immediately after treatment.

We use both superficial and medium depth peels at Privée and the peels are tailored to your needs. Generally the more aggressive the peel, the more obvious the results.

6) Radio frequency (RF) resurfacing

This treatment is used primarily for increasing the production of collagen in the skin, which helps minimise the look of pores, improve texture and overall improve the complexion. The treatment ablates tiny fractions of the skin using RF as the energy source. This triggers the body to go into repair mode, increasing the production of collagen. The increase in collagen makes the pores appear smaller and the skin tighter. Down time is usually less than 24 hours with most people being able to resume their normal activities the next day. The skin will feel dry for a week post treatment. We recommend a course of 4 treatments spaced 4-6 weeks apart.

7) Skin Needling

Skin needling is a minimally-invasive procedure that involves the use of a micro-needling device to create a controlled micro skin injury. These controlled ‘micro-injuries’ in the skin trigger new collagen synthesis, the result is smoother, firmer and younger looking skin. While a perfect skin-care routine will work for you year round, there are some important steps you should keep top of mind as the weather gets warmer.

Warm weather brings with it skin-care challenges and questions like, " Do I really have to moisturise even if it's hot and my face feels oily? ” (btw - the answer is, Yes!)

It’s also important that sunscreen is re-applied every couple of hours, so make sure to pack extra sunscreen for those longer summer days outdoors (at Bondi Beach).

Use lighter weight skincare products

In the Summer, I generally remind all my clients that their skincare routine should be a little more lightweight. Consider combining your sunscreen and moisturiser for a lighter weight alternative to avoid the clogging of pores on your face. Also, the heat and humidity means you can swap out your heavier cleanser in favour of a foaming option. For example, our favourite cleanser at the Clinic is the Advanced Cleanser from Dermaceutic.

Vitamin C Cream

Vitamin C is great to use year-round, but super important in the summer. Vitamin C is proven to brighten and even your complexion as well as providing significant anti-ageing benefits. It can also help with collagen production. Collagen is an essential protein required for healthy youthful looking skin. Put a few drops on your skin between cleansing and moisturiser. My favourite Vitamin C product is CE Ferulic which is a potent antioxidant, pigment inhibitor and skin brightener.

Exfoliate

Exfoliation is a necessary step to maintaining that dewy and healthy glowing skin. It's especially helpful if you suffer from acne, blocked pores or pigmentation. Chemical exfoliants work best; look for products such as Salicylic, Mandelic, Kojic and Azelaic acid. Cut down your showers and bath time

Having long showers in water that is too hot in Summer can lead to over-drying leading to inflammation, even eczema. So keep those showers and baths shorter, and the water temperature warm to cool. Plus it saves water!

Stay in the shade

What is my skin type?

Everyone’s skin is different and reacts differently to the sun. Ever wondered what type of skin you have? Nobody has the exact same skin as you. But you still fall into one of the major categories. Once you work out how to characterise your skin, you can choose the correct skincare.

The Fitzpatrick scale is a recognized tool for dermatological research into skin types. It was developed as a way to estimate the response of different types of skin to ultraviolet light (UV). This scale places the skin in one of six categories, based on how much melanin is present. Type 1 being pale skin that burns without tanning & Type 6 being darker skin that never burns and only tans.

Note that these 6 categories are just a guide, as some individuals may fall between two.

What does the Fitzpatrick scale mean for my skin ?

Type 1 and 2

Those with lighter Type 1 & 2 skin (often redheads and fair-skinned individuals) should always be wearing a broad spectrum SPF sunscreen and avoid the sun as much as possible. This is because lighter skin has less melanin, making it more difficult to naturally block harmful rays from the sun. The ability to tan is almost impossible and therefore these skin types are much more sensitive and are at higher risk for skin cancer.

Type 5 and 6

An individual with a level 5 or 6 Fitzpatrick skin type would typically be someone with Indian or African heritage. While it is still recommended to use sun protection at all times, these skin types have a much lower risk of developing skin cancers & photo-aging due to the high levels of melanin and the decrease in sensitivity to the sun’s UV rays.

Products and treatments depending on your skin type

The type of skin you have determines which treatments and products will work best on your skin, and what you should be avoiding.

Depending on what Fitzpatrick skin type you are, will determine which medical-grade skin treatments are appropriate. Because those with darker skin types are more able to produce pigment, any skin or pigmentation treatment that uses high levels of heat to induce a response may be risky. For example, certain lasers are not recommended for Fitz skin types 4-6 because the risk of post-inflammatory hyperpigmentation (PIH) is increased. PIH results in the skin producing rebound pigment in response to trauma such as heat. Often this will happen months after the initial trauma.

Patients with darker skin types will often need to prepare their skin on lightening agents prior to undergoing more intense treatments with products called Tyrosinase Inhibitors. The Radiance brightening cream or the Mela Cream from Dermaceutic are great skin prepping options.

Common skin type categories

Beyond the Fitzpatrick Scale, there are 4 common categories for the type of skin you have. Oily, Dry, Combination and Normal. These are most commonly used by skincare and cosmetic companies.

To determine which of these you have, wash your face, don’t apply anything to the skin and wait 2 hours. Your skin may display some of these signs:

Oily Skin - The skin remains oily and shiny all over. Skin tends to be more acne-prone.

Dry Skin - Rough and flaky on the face. The skin feels tight.

Combination - Your t-zone is oily or shiny, but there is some tightness and dryness around the cheeks/jawline

Normal - The skin feels smooth with no excessive dryness or shine.

Depending on where you fall in this category, will guide product choices and the active ingredients which will be most suitable for your skin.

Anti-ageing treatments for the décolletage region

The decolletage (dec) refers to the chest area, below your clavicle bones and just above the breast. The skin on your neck and dec are quite different to that of the face, making them more prone to signs of ageing. This is because the skin on both of these areas is thinner, has less fat and there are very fewer sebaceous glands here. The reduced amount of oil can create more dryness and the thinner skin is more likely to show the signs of ageing such as reduced skin laxity and crepey texture. The reduced oil in this area can also make the skin more delicate and there is an increase in healing time which needs to be taken into consideration when undergoing invasive treatments. There can be an increased risk for scarring including keloids in this area comparatively to the face.

What treatments work best for the decolletage region?

This depends on what exactly you are trying to treat. Is it for pigmentation, vascular lesions, to diffuse redness or to improve the texture and crepey appearance, wrinkles or lines?

1. Radio Frequency (RF) Skin Resurfacing using the Venus Viva machine

Treats best- Stimulating collagen and elastin for improvement of texture, lines and wrinkles.

Treats moderately - Pigmentation (maybe a safer option for darker skin types (4 and above) and long-term improvement of rosacea and redness.

Venus Viva RF uses multipolar radio frequency to stimulate the skin’s cells (fibroblasts). The machine uses metal pins and RF heat to fractionally ablate tiny microscopic holes in the skin. We can control the power and depth of each ablation. This microtrauma causes a series of events in the wound healing cascade which eventually lead to an increase in the production of collagen and elastin, repairing damaged tissue and remodelling existing collagen. The overall effect is the skin rebuilding itself from the inside out, filling in acne scars, lines, and wrinkles, skin tightening, and improving hyperpigmentation and discolouration. It’s a great option for targeting multiple ageing concerns such as tightness, texture, lines, pigmentation and redness. It works well for those vertical lines that develop on the chest due to excessive UV damage.

RF works well on the decolletage region as it is gentle enough for the skin whilst still producing effective results. Typically 4 sessions are needed to be spaced 4 weeks apart for maximum results and the treatment is suitable for all skin types.

2. Skin Needling

Treats best - Collagen stimulation and improvement of texture.

Treats Minimally - Pigmentation

Skin Needling is another popular treatment for the decolletage region. Using tiny needles to create microscopic wounds in the skin, needling stimulates collagen production, helping to improve the overall quality of the skin.

Skin Needling can work well however more treatments will be needed compared to RF and it does not treat pigment as well unless lig0htening ampoules are infused into the skin post-treatment. These can be purchased as an add-on treatment. Skin needling will not address redness in the skin from sun damage. 6-8 treatments are recommended spaced 4-6 weeks apart.

3. IPL/BBL (Intense Pulsed Light/Broad Band Light)

Treats best- Pigmentation and redness (from sun damage)

Treats moderately - Collagen stimulation

It does not treat - Hormonal pigmentation or skin types 4 and above

IPL is a light-based treatment. Each pulse of light contains multiple light wavelengths each attracted to different chromophores or colours in the skin. The dec area is highly sensitive to sun damage as it is often not covered or protected by SPF. The IPL/BBL will clear the red and brown tones from the skin. Lower treatment settings are used and more sessions may be needed compared to the face due to the reduced healing capacity of this area. A minimum of 4 treatments is recommended, spaced 4 weeks apart. Treatment can be done all year if the area is protected from the sun. Darker skin types are better suited to using RF.

4. Chemical Peels

Treats best - Superficial textural issues and pigmentation

Here at Privée Clinic, we offer varying intensities and types of chemical peels for the decolletage region.

For lower-intensity peels (with less downtime) we offer the Mesoestetic range which stocks Glycolic, Salicylic, Lactic, and Modified Jessner Chemical Peels. Your practitioner will choose the type of peel based on your goals, skin type and amount of downtime. A series of 6-8 treatments are recommended monthly using superficial peels.

For more intense results we also offer Trichloracetic acid (TCA) peels from Dermaceutic. TCA is a powerful anti-aging ingredient that targets the upper dermis and stimulates collagen production.

We can adjust the percentage of TCA used for a more intense peel that will result in heavier peeling and longer downtime or a lower percentage which will reduce the healing time but secondary treatments may be needed.

Treating the décolletage region can be effective as long as the skin type is taken into consideration and in general lower treatment settings are used.

How to determine what type of skin you have

Have you ever gone to the store and tried to purchase some skincare, but get so overwhelmed by the different types and uses? Understanding your skin type and colour can be frustrating, especially as skin is constantly changing depending on our environment, age, hormones, and what current products are being used.

The first way to classify skin is by using the Fitzpatrick Scale. Developed in 1975 this is a way to classify skin based on how it reacts to the sun. This scale places the skin in one of six categories, based on how much melanin is present. One being pale skin that burns without tanning & six being darker skin that never burns and only tans.

Note that these 6 categories are just a guide, as some individuals may fall between two.

So, what does the Fitzpatrick Scale mean for me?

Those with lighter Type 1 & 2 skin (often redheads and fair-skinned individuals) should always be wearing a high SPF sunscreen and avoid the sun as much as possible. This is because lighter skin has less melanin, making it more difficult to naturally block harmful rays from the sun. The ability to tan is almost impossible and therefore these skin types are much more sensitive.

The type of skin you have determines which treatments and products will work best on your skin, and what you should be avoiding. Depending on what Fitzpatrick skin type you are, will determine which medical-grade skin treatments are appropriate. Because those with darker skin types are more able to produce pigment, any skin or pigmentation treatment that uses high levels of heat to induce a response may be risky. For example, certain lasers are not recommended for Fitz skin types 4-6 because the risk of post-inflammatory hyperpigmentation (PIH) is increased. PIH results in the skin producing rebound pigment in response to trauma such as heat. Often this will happen months after the initial trauma. Patients with darker skin types will often need to prep their skin on lightening agents prior to undergoing more intense treatments with products called Tyrosinase Inhibitors.

Beyond the Fitzpatrick Scale, there are 4 common categories for the type of skin you have. Oily, Dry, Combination and Normal. These are most commonly used by skincare and cosmetic companies.

To determine which of these you have, wash your face, don’t apply anything to the skin and wait 2 hours. Your skin may display some of these signs:

Oily Skin - The skin remains oily and shiny all over. Skin tends to be more acne-prone.

Dry Skin - Rough and flaky on the face. The skin feels tight.

Combination - Your t-zone is oily or shiny, but there is some tightness and dryness around the cheeks/jawline

Normal - The skin feels smooth with no excessive dryness or shine.

Depending on where you fall in this category, will guide product choices and the active ingredients which will be most suitable for your skin.

How to treat an ageing neck

As we get older, the neck is often one of the first areas to really show our age. The skin in this area is much thinner and is therefore prone to ageing. We often forget to apply sunscreen to this area focusing on the face and this delicate skin gets forgotten. The skin loses elasticity and begins to sag, or we develop a small pocket of fat in the submental (under chin) area. Factors such as genetics, weight gain or loss, photo-ageing from UV exposure and overactive platysmal bands can all contribute to how the neck ages. The quality of the skin, the degree of skin ageing, skin laxity and the amount of submental fat present will guide the treatment options. Treatment options for neck ageing are categorised by skin laxity, neck adiposity and platysmal banding.

Treatment options for a fuller neck with good skin quality and minimal skin laxity include Multi-polar RF skin tightening to break down fat cells and rebuild collagen and elastin, and fat dissolving injections to reduce neck fullness.

For a neck with minimal sub-mental fullness but with skin laxity and loss of collagen then treatment options include Multi-polar skin tightening, Radio-frequency resurfacing, PDO mono threads to tighten and stimulate collagen production, anti-wrinkle injections to the platysmal bands and skin boosters to the neck wrinkles.

Diamond Polar (Multipolar) Skin Tightening - As we age we lose collagen and elastin, leading to wrinkles, sagging and necklace lines. Venus Viva diamond polar applicator uses heat to naturally boost collagen production to tighten, lift and reduce fine lines and wrinkles. This treatment is perfect for firming and lifting loose skin on the face and neck. It’s technology helps to sculpt the contours of the face and neck, providing a more youthful appearance. A gel is placed over the neck area to be treated and the skin is heated up to between 39-43 degrees celcius by running a specialised handpiece over the area for approximately 30 mins. At these higher temperatures, the multipolar handpiece can help permanently break down the fatty deposits under the chin and jawline. The treatment has no downtime and pain, and generally takes 30 mins. Typically we recommend 1 session per week for 6 weeks for optimal results.
Sub-Mental Fat Injections - If you have a pocket of fat in the neck or under the chin, creating excessive fullness, submental fat injections can work wonders. These injections reduce fat under the chin, resulting in a more contoured neck profile and jawline. The solution is injected into the pre-platysmal fat layer, between the dermis and the platysmal muscle. The solution breaks open the fat cell membrane, the fat leaks out into the tissues and is engulfed by circulating white blood cells via the process of inflammation. It is then taken to the liver via the lymphatics where it is metabolised and then eliminated from the body. Once the fat cells have been destroyed, they are gone forever.

Sub-mental fat injections have approximately 1 week of swelling and downtime due to the fact that the drug works through the process of inflammation. The number of injections and the number of treatments are tailored to the individual patient’s submental fat distribution and treatment goals. A minimum of 2 treatments is usually required. Common side effects include swelling, bruising, temporary numbness, itchiness and skin tightness.
PDO Mono Threads - If you have minimal fat under the chin but have a loss of elasticity and loose skin, then PDO Mono Threads are a fantastic treatment for rebuilding collagen and tightening the neck area and defining the jawline.

PDO Mono Threads work by injecting a thread made from polydioxanone which is an absorbent polymer, (the same material used in dissolvable sutures). They are a flexible and durable thread that is inserted into the face, neck or body, stimulating the formation of collagen around the thread and giving support to the tissues and a tightening effect to the skin.

The thread itself lasts approximately 6 months before it gets reabsorbed by the body. The collagen built around the thread, however, will last for approximately 12-24 months. Post-treatment, there may be some bruising and tenderness that lasts for up to a week.

Neurotoxins to the platysmal neck bands - A large sheet of muscle called the platysma covers the neck and extends up over the mandible. This muscle over time, caused by excessive contraction, can form vertical neck bands which give the appearance of a saggy turkey neck. Clinical studies have shown that allowing these platysmal bands to go untreated can speed up neck ageing. Neurotoxin injections are injected superficially down each vertical neckband to relax the muscle and give a more contoured and smooth appearance
Skin boosters - These are thin hydrating fillers that are injected, either with a cannula or needle, into the dermis of the skin. They are composed of a sugar that is naturally occurring in the body and are hydrophilic in nature, which means water is attracted to the gel, creating intense hydration of the skin. Skinboosters are less volumising and more hydrating and are used to improve the textural appearance and quality of the skin. In the neck, these fillers can be used to fill necklace lines or to improve the general appearance of skin laxity.

Why Winter is the best time to tackle your pigmentation

Coming into the colder winter months is the perfect time to look at what de-pigmentation treatments you can do and begin tackling sun spots, freckles, melasma, and more.

This is the best time of year for these treatments as the weather is cooler and we tend to spend more time indoors. There are several critical reasons why you should avoid the sun when treating pigmentation.

Less sun exposure - This is critical as after any depigmentation treatment your skin becomes incredibly sun-sensitive, similar to that of a sunburn. If you expose your skin to harmful UV rays post-treatment you can potentially affect the results of your treatments and cause the pigment to return.
The skin is usually less tanned due to the decrease in sun exposure - It's incredibly important to not use/apply de-pigmentation treatments on tanned skin (both natural or fake tan) as this can lead to increased risks of burns, post-inflammatory pigmentation (PIH) and side-effects.

When it comes to de-pigmentation, we offer several different treatments and approaches to suit every individual's lifestyle and budget.

Cosmelan - Our most popular depigmentation treatment. The topical application targets pigmentation and melasma through a deeply penetrating mask applied in the clinic and washed off at home after 8-12 hours depending on your skin type. It significantly reduces the blemishes caused by increased Melanin production by gently fading the pigment and suppressing further pigment from resurfacing. How to slim your face with injectables – 5 different non-surgical techniques.

For my fellow full faced friends, there are actually many non-surgical treatments available today that can be implemented before needing to go under the knife. Using injectables we can sculpt and contour the facial shape creating a more balanced, slimmer and chiselled look. Here are the top 5 ways to achieve a slimmer, more sculpted face.

Anti-wrinkle injections (neurotoxin) into the masseter muscle

The masseter muscle is a large rectangular muscle which originates from the lateral cheek bone (zygomatic arch) and inserts into the angle of the jaw and along the lateral surface of the mandibular ramus (posterior part of the jaw). It is the main muscle we use in order to chew our food. However, some people develop overly large masseter muscles due to teeth grinding, teeth clenching, chewing tough food or chewing gum. Some cultures genetically have naturally large masseter muscles.

Over time with constant contraction of this muscle, the muscle can become quite bulky and cause widening of the lower face. This is not so much of a problem in men because a wide, square jaw is desirable. However, in women, an oval shaped face is generally the most aesthetically pleasing, and by reducing the bulkiness of this muscle, it can dramatically slim the lower face. Injections of neurotoxin are placed deep in this muscle and within 6-8 weeks the muscle is reduced, and the lower face shape is slimmed. It’s important to keep the injection placement in the posterior part of the muscle in order to avoid other muscles and smile asymmetries.

Cheek fillers

Most people think that adding more volume to the cheeks will create more fullness, however if filler is strategically placed on the lateral cheek bone, this can give the illusion of a slimmer face. Obviously if the patient is already full in the lateral aspect of the cheek then this will not be necessary, however those with a round face often find that they lack structure, especially in the lateral cheek bones. Creating structure here will give more height to the zygomatic arch and give a more chiselled appearance. Using a thick filler with a high G prime will allow for a sharper look.

Chin filler

Adding filler to the chin for projection can elongate the jawline, again giving the illusion of more depth, rather than a round, full face. Many people have slightly recessive chins and therefore can do with a slightly more projected chin. Adding volume in this area will also make the double chin appear less obvious. In women we can give the illusion of a narrower chin by adding filler to the midline of the chin, creating more of a v shape. The ideal chin width in women to create a more feminine appearance is usually the same as the distance between the inner corners of the eyes (inner canthus). Again, men usually want a wider chin as this is more masculine and the chin should be the same width as the mouth.

Jawline filler

Adding filler along the jawline can camouflage jowls and again create length, which will counterbalance the round fuller face. By slightly accentuating the angle of the jaw and along the mandible with filler, we can create long sleek lines which will give structure creating the illusion of a slimmer face.

Sub-mental fat dissolving

Fat dissolving injections can be placed into the turkey neck or double chin (submental fat). These injections work much like a detergent, they break open the fat cell membrane, the fat leaks out into the tissues, and the body’s white blood cells come to the site where they start engulfing the fat and take it to the liver via the lymphatic system where it is metabolised and eliminated from the body. This process can take up to 12 weeks, but the results are permanent. Normally at least 2 treatments will be needed and can be done 4-6 weeks apart.

With summer just a memory, now is the time to fade sun spots and reverse visible signs of sun damage with a course of IPL at Privée Cinic. Ideal for lightening non-hormonal hyperpigmentation and evening out the complexion, Intense Pulsed Light (IPL) is one of our most popular treatments during winter. And for good reason. Read on to discover everything you need to know about IPL and its rejuvenating benefits.

What is IPL?

Intense Pulsed Light or IPL refers to the use of light energy for skin rejuvenation, hair removal, and to treat specific skin concerns such as acne. As short bursts (‘pulses’) of intense light are administered to the surface of the skin, thermal energy penetrates into deeper skin tissue to performs tasks such as breaking down pigment, boosting collagen production, and constricting blood vessels to improve the appearance of broken capillaries.

What is IPL used for?

IPL’s first application was in permanent hair reduction, but it is now also considered an effective, non-invasive treatment for skin rejuvenation. As well as addressing redness and vascular concerns (spider veins, broken capillaries), it can be very beneficial for those with active acne as well as scarring. It is also used for skin tightening, addressing enlarged pores, and wrinkle reduction. However, the usage we’re highlighting post-summer is IPL for sun damage.

How does IPL reduce sun damage?

General sun damage, freckles and sunspots can be successfully treated with IPL, with the best results achieved on fair-skinned individuals with areas of dark pigment (the same goes for hair removal – dark hair and pale skin is the most suitable combination). Quick flashes of light target areas of excess melanin, causing them to heat up, break down and flake off (or be absorbed by the lymphatic system), in a process known as Photothermolysis.

After the initial IPL treatment, pigment in the treated areas turns quite dark and will remain darker for the next week or so, until gradually flaking off. The majority of the pigment is gone after the first treatment, however second and third treatments are typically needed to mop up the remaining bits of pigment and further lighten the skin.

Can anyone have IPL?

IPL can be used on most skin types, but is usually unsuitable for darker skin tones. In these individuals, more treatments are required at a lower setting in order to reduce the risk of post-inflammatory hyperpigmentation (PIH). A course of peels is therefore considered a safer option for treating sun damage. IPL is not recommended for melasma (hormonal hyperpigmentation), as the condition can be stimulated by the heat and become worse. ( Cosmelan , on the other hand, is very successful at improving melasma.)

IPL is also contraindicated for pregnant women, those with diseases that are stimulated by light (eg epilepsy), and for those on photosensitive drugs, such as Roaccutane.

Treatments required & downtime?

For IPL for sun damage, we recommend three sessions spaced about one month apart. The treatment is not painful, but has a sensation similar to a rubber band snapping on your face. Any discomfort is minimised with the application of a cooling gel pre-treatment. There is no downtime, and you can immediately return to your daily activities.

The Collagen Stimulating Facelift: Everything You Need to Know

Looking for a tighter, firmer, younger-looking face but not keen to go under the knife? Our Collagen Stimulating Facelift utilises filler to give results similar to a facelift, minus the surgery! In this rejuvenating technique, semi-permanent liquid filler is injected all over the face, deep into the tissues, where it stimulates the production of collagen and elastin, thereby building volume, sculpting, tightening and lifting. The added bonus? This product will also improve skin texture and elasticity – something a surgical facelift will not do.

These fillers are different to gel-based fillers in that they stimulate the body’s cells to make more collagen, rather than act as a gel implant that attracts water to create the volume.

What does the Collagen Stimulating Facelift involve?

The group of fillers used in the Collagen Stimulating Facelift are derived from fruit acid (the same material in dissolvable sutures). The filler comes in powder form and is reconstituted with water and lidocaine at least 48 hours prior to treatment to make sure the product is completely dissolved and allows for ease of injection. The added lidocaine significantly reduces the pain associated with the treatment, and topical anaesthetic cream is also applied prior to injection to ensure comfort.

This group of fillers builds the collagen slowly over a period of up to 6 months, and 3-4 treatments will be required every 4-6 weeks during this time. The results of the first treatment may be noticeable after the first month but patience is a virtue. It is a much slower build than with gel-based fillers (used in the Liquid Facelift ); however, when the results kick in they can be quite amazing and long lasting!

How many vials are required?

It is recommended to treat the whole face because, as the treatment kicks in, the skin in that area improves. If only certain areas are injected, the difference in skin quality may be more noticeable.

The general rule of thumb is, whatever decade the person is in, that is how many vials will be required. So, for someone in their 50s 5 vials in total may be required over a period of 3-4 months. For a younger patient, only 2 vials may be required – it just depends on how much volume loss there is and what areas require building up. If a patient with no cheekbones wants the razor-sharp supermodel cheekbone, then more vials will obviously be required.

What are the side effects or risks?

The immediate side effects of the treatment include redness, swelling and potential bruising. These are temporary and will resolve within 72 hours. Bruising may take up to 1 week or more to resolve, depending on the patient’s clotting profile and genetics.

When injected correctly into the right tissue plane, these treatments are very safe. Many years ago, there was a high risk of nodules; however, due to changes in how we reconstitute the product (it’s now mixed with more fluid and less concentrated), this risk is almost negligible. It is, however, important to go to an experienced injector as these fillers are not able to be dissolved.

Risks are nodules, infection, delayed skin reactions, granuloma, scarring, necrosis and – in extremely rare cases – blindness. The treatment is contra-indicated in patients with auto-immune disease because of the increased risk of delayed onset reactions and granulomas. In these patients, the immune system may see the filler as a foreign body and cause an acute inflammatory response. The same risks can be experienced with other gel-based fillers. However, as these liquid fillers cannot be dissolved, the overall risk is probably slightly higher than with the gel-based fillers.

What is the cost?

The cost of the Collagen Stimulating Facelift at Privée Clinic is $880 per vial, or $1,500 for 2 vials. Overall the treatment is a long-term solution to improve the skin quality, tightness and volume. Because the whole face is addressed and the fillers used are long-lasting, this treatment is quite cost effective. Much less invasive and minus the downtime of surgical treatments like Liposuction, here’s everything you need to know about sub-mental fat injections.

What are sub-mental fat injections?

Used therapeutically for over 15 years, sub-mental fat injections were first used as a double chin treatment in 1995 by Brazilian physician, Dr Rittes. Primarily used to ‘melt away’ fat under the chin and redefine the jawline, the treatment involves a liquid lecithin solution being injected into the subcutaneous layer of the skin. The solution, derived from soya beans, breaks down the fat cell membrane, allowing fat to leak out and be transported to the liver via the lymphatic system. Here, it is metabolised and eliminated from the body.

Once the fat cell has been destroyed, it is gone forever. However, this doesn’t mean you should break out the doughnuts! A healthy lifestyle is important to avoid weight gain overall, as well as in your problem areas.

Can the treatment be used to reduce fat in other areas?

While most commonly used to treat a double chin, sub-mental fat injections can also be used successfully in other small areas such as the knees, stomach, love handles, inner/outer thighs, or saddle bags.

Who are these double chin injections suitable for?

Sub-mental fat injections are no substitute for a healthy diet and adequate exercise, and they are generally not suitable for overweight individuals. They can, however, be an incredibly effective treatment for people of a healthy weight concerned with small pockets of stubborn fat that do not respond to lifestyle changes.

Most healthy individuals can undergo the treatment; however, it is/may not be suitable for pregnant women, diabetics, people allergic to soy products, or those with vascular, liver or renal disease. Click here for the full list of contraindications.

How does this double chin treatment compare to others?

With Liposuction, there is a higher risk of defects in the contours of the skin, i.e. unevenness resulting from too much fat being taken. There is also more pain and, of course, the potential risks associated with surgery and sedation. Sub-mental fat injections with lecithin are also less invasive than some other fat dissolving injections, and typically yield a more even contour.

On the downside, results are less immediate than with surgery – it will take approximately four weeks before results start to become visible. Also bear in mind that the treatment involves significant swelling, since it works via inflammation.

What can I expert from the treatment?

The double chin reduction treatment will take around 20 minutes, with up to 40 injections administered. Local anaesthetic is mixed with the product to reduce the pain associated, with most people able to tolerate the procedure. As the breakdown of fat occurs through the inflammatory response, there is a considerable amount of swelling starting immediately during/after the treatment.

Other normal side effects that can be expected are warmth at the injection site, bruising, tenderness and sensitivity to pressure and pain in the treated area. Swelling typically remains for a minimum of one week, with most people able to return to work the day after treatment. Our tip: wear a scarf if you’re sensitive about your temporary turkey neck!

It will take approximately four weeks before results start to become visible. Be patient, as dramatic results can been seen if the recommended treatment regimen is followed.

How many treatments are required?

For double chin reduction, a minimum of two treatments, but typically three to four, is required. These are typically placed one month apart.

One of the most common beauty complaints, dark circles can undoubtedly be a drag. Whether you blame yours on your mother or your party-hard lifestyle, under-eye concealer is not the only solution available. Because, while tricky to treat, dark circles can be improved. Read on to discover how.

Dark circle causes

Dark circles in the skin around the eyes can be caused by the underlying blood supply, pigmentation from sun damage, or under-eye hollowness. A combination of genetics, thin skin, pale skin, poor circulation and environmental factors such as lack of sleep, alcohol, poor diet, allergies and toxins can cause dilation of the blood vessels and sluggish blood flow in this under-eye area.

Hollowness under the eyes can also cast a shadow and create the illusion of darker circles, while superficial veins in the area can also increase the look of darkness.

Finding the right injector is key

To find the practitioner that’s right for you, ask around for recommendations, look at reviews, and look to social media, where most injectors will post examples of their work. Also take note of what her, or his, lips look like. Chances are, if your injector is overdone, you might not walk out with the kind of subtle boost you’re after!

Allergy risk is low

Today’s lip fillers are made from Hyaluronic acid – a naturally occurring substance in the body found in skin, joints and synovial fluid. Because of this, the risk of filler-related allergy is low.

Lip fillers are reversible if you’re unhappy

If you’re not happy with your results, you can easily deflate your lips. A product called hyaluronidase is injected, containing an enzyme that breaks down the Hyaluronic Acid within 24 hours. However, there are some downsides. As hyaluronidase is derived from bovine sources, some people can be allergic to it. It also will temporarily break down native HA in the skin. It increases the cell permeability and spreads through the tissues relatively quickly.

They’ll last 6 months or more

Depending on the type of filler used, the particle size, and the amount of cross linking in the Hyaluronic Acid, your lip filler could last anywhere from 6-18 months! Those with a faster metabolism will also metabolise the HA faster, meaning they will need more frequent top ups.

They should feel quite natural

Your lush new lips should feel natural – they’ll feel firmer for the first week then soften to feel like natural tissue. Some products are firmer than others, but in certain people firmness is required to give structure and definition back to the lip.

It’s a super speedy treatment

In around 20 minutes, you’ll be out the door.

With minimal downtime

The downtime for lip fillers is minimal, especially if you follow the instructions of your injector to prevent swelling/ bruising. In the majority of cases, any swelling and tenderness will be resolved within 24-48 hours.

Lip fillers aren’t too painful

Topical anaesthetic cream is applied first, and there is local anaesthetic in the product. There is a small degree of discomfort and a sting upon injection, however the lips become numb relative quickly once a little product is injected into the area.

You need to avoid some things beforehand

To reduce the risk of bruising and/or swelling, it’s advisable you avoid fish oil, Vitamin E, anticoagulants, and certain Chinese herbs for a week before your treatment. Avoid alcohol for 24 hours before and 48 hours afterwards. Potential swelling can be reduced by applying ice after the treatment.

You should skip the lipstick for a bit afterwards

After your lip injections, avoid touching your lips too much, and do not apply makeup or lipstick to the area for 24 hours. Also, stay out of the sun and avoid hot drinks while the lips are still numb.

Infection is a rare, but possible side effect

In terms of more serious side effects, infection is a possibility, albeit a rare one. It’s therefore iportant to have clean hands when touching your lips after the treatment. Accidental injection into a blood vessel during the procedure can cause necrosis. Again, this is rare, and the filler would need to be dissolved in this case.

You might need more than one treatment

If you’re wanting significantly fuller lips, multiple treatments are needed, close together, to achieve your desired results.

1. Hyperhydrosis (excessive sweating)

Anti-wrinkle injections in an area prone to sweat inhibits the signal from the brain to the muscle needed to release the sweat. We often treat underarms for this problem, but palms and feet can be injected too. Firstly, a grid like pattern is marked out in the area, then a hyper-diluted solution of neurotoxin is injected superficially into the skin covering the area. Within a week the patient will be sweat free!

2. Teeth grinding

We treat a lot of people for painful teeth grinding at Privée. By delivering an anti-wrinkle injection to the masseter muscle in the jaw, we are able to reduce the ability to clench and grind teeth. This limits the excessive muscle activity which in turn stops the person from grinding. Injections in this area usually last 6 months. A side benefit of this can be the softening of a square jawline.

3. Weak jawline

We call this the Nefertiti Lift. Injections are placed along the length of the jawline to relax the muscle and give more definition to a jawline.

4. Resting Bitch Face

There is a muscle which attaches to the corner of the mouth and extends down to the jaw. Normal contraction of this muscle can pull the corners of the mouth down, creating that unwanted Resting Bitch Face appearance. By injecting and relaxing this muscle, we can subtly lift the corners of the mouth.

5. Orange peel chin

When the muscle on the chin contracts (mentalis muscle), it can give the surface of the skin a strange orange peel look. Over time this will cause superficial lines and wrinkles to become permanently etched and also create a deep horizontal line between the chin and the mouth, called a mental crease. Anti-wrinkle injections into this area will smooth the skin, rejuvenating the area.

6. Brow lift

When anti-wrinkle injections are placed just under the eyebrows and into the frown muscles, a brow lift can be achieved. Normally when these muscles contract they pull the whole brow down. When relaxed with a neurotoxin, the muscle cannot pull down and therefore the brow lifts.

7. Gummy smile

If you show a lot of gum when you smile, it’s usually because you have overactive muscles around your nose and mouth, causing the lip to be drawn too high. Injections into the muscles just beside the alar base of the nose can improve your smile by lessening the excessive pulling up of the lip.

8. Drooping nose tip

If your nose tip moves when you talk or droops when you smile, then anti-wrinkle injections in the base of the nose can stop the movement and help to lift the tip.

9. Ageing neck

The neck can quickly give over our true age! The neck is covered in a muscle which can form long stringy bands during contraction. These look unsightly, often contributing to the turkey neck and a less than defined jawline. Injecting into this muscle can rejuvenate this area.

10. Disappearing top lip

Many people lose their top lip when they smile, as the muscle contracts and the lip rolls inwards. Injections can be placed just above the border of the lip to stop the lip from doing this. It slightly everts the lip, and gives added plumpness.
